Your GI MAP Paperwork includes:

1. Test Requisition Form
   Fill out the following information:
   – Patient Information section
   – Test Information section: Date Sample Collected (GI-MAP should be selected, as well as Zonulin add-on if you are testing for gut permeability)
   – Credit Card Payment Authorization section (unless you have paid through the Clinic)
2. Commercial Invoice (X3)
   Fill out the following information:
   – Ship Date (top right hand corner)
3. FedEx Shipping label
   – Section 1: Your information
   – Section 3: Shipment Information
   – Section 8: Signature

The test requisition form should be placed in the outside pocket of the specimen bag

The commercial invoices (there should be 3) and the FedEx shipping label should be in the purple FedEx pocket that you attach to the FedEx shipping bag.
